Toast pistachios in a dry skillet over medium heat until fragrant, about 5 minutes.
Let pistachios cool, then blend until finely ground.
In a saucepan, combine milk, heavy cream, and sugar; heat over medium until sugar dissolves, stirring occasionally.
Whisk egg yolks in a bowl until pale and thick.
Slowly pour warm milk mixture into egg yolks, whisking constantly to temper the eggs.
Return the mixture to the saucepan and cook over low heat, stirring until thickened enough to coat the back of a spoon, about 5-7 minutes.
Remove from heat and stir in ground pistachios, vanilla extract, and salt.
Cool the mixture to room temperature, then refrigerate until chilled, about 2 hours.
Churn the mixture in an ice cream maker according to manufacturer instructions until thick and creamy.
Transfer to an airtight container and freeze until firm, at least 4 hours.
Serve scoops of pistachio ice cream and garnish with chopped pistachios if desired.
