Melt about 1 tablespoon of butter in a fairly large pot or pan over low heat.
Add 3 tablespoons of plain wheat flour to the melted butter and stir continuously for about 5 minutes to cook the roux until it has a biscuity flavor.
Gradually add about 200 mL of full-fat milk to the roux, stirring until it thickens and becomes smooth.
Continue to add more milk, stirring for another 2-3 minutes until the mixture thickens further.
Add a pinch or two of nutmeg and black pepper to the mixture, adjusting for salt if desired due to the salted butter.
Raise the heat slightly and add a total of 800 mL of milk, stirring continuously for about 3-4 minutes until thickened.
Stir in 2 cups of grated cheddar cheese until melted and combined into the sauce.
Add about 6 cups of cooked macaroni to the cheese sauce and mix well, cooking for an additional 2-3 minutes.
Optionally, transfer to an oven-proof dish, top with more cheese, and bake, or serve as is with a sprinkle of cayenne pepper on top.
